The Application Form is in Adobe Acrobat (PDF) format. Using the Acrobat Reader, you may  download the Application Form, print the form, complete it, and mail it along with your *$40.00 application fee to:

If you are applying for any of the Counselling programs in the seminary you will need to complete a Personal Assessment Interview conducted by an assigned Counselling Faculty member for Academic and Professional purposes. This will be done either in person or by phone depending on your location of study. This interview will be conducted within the first three weeks of either the Fall or Winter semester depending on when your studies first commence.

Apply to the Doctor of Ministry program.

*Please note: If you are applying for the Fall Semester after May 1 or for the Winter Semester after October 1, then the application fee increases to $60.00.

Scholarships and Bursaries

Providence Theological Seminary offers a variety of scholarships and bursaries to reward academic achievement and assist in financing the cost of education. All scholarship applicants must meet the regular criteria for admission to Providence Theological Seminary and are encouraged to apply as soon as possible. To apply for these scholarships please download the  Providence Theological Seminary Bursary/Scholarship Application form or contact the Admissions Office.

How to complete this Application Packet

We want to process your application as quickly as possible. Here’s a checklist to help you complete the application process.

1. Complete the application form and return it to us in the post-paid envelope.
2. Attach two recent wallet-size photographs of yourself to the application form.
3. Canadian students enclose the *$40.00 application fee. International students enclose the $150.00 (CDN) application fee. Cheques may be written to Providence Theological Seminary.
4. Write the autobiographical essay and enclose it with the application (typewritten preferred).
5. Complete the housing form and enclose it with the application.
6. Complete the applicant’s portion of the medical form. If any of the conditions listed at the bottom of the form apply, then have your doctor/counsellor complete the remainder and send it to the Admissions Office.
7. Give the four reference forms to the following people:
   • Your pastor or church staff person who knows you best
   • A Professor or employer (if you have been out of school for more than three years)
   • A friend
   • A friend
8. Request a transcript of your grades from any post-secondary institutions you have attended.
   Note: transcripts must be sent directly from your school.
