To order official or unofficial transcripts, print this form and send it to Providence College and Theological Seminary as directed above. Transcript fees must be paid before the transcript is prepared. Official transcript fees are $8 each. There are additional charges for faxing and couriering transcripts. They bear the signature of the Associate Registrar and the seal of the institution. Unofficial transcripts are free and do not bear a signature or seal.
